ATS Preparticipation Requirements

ATS PRE-PARTICIPATION REQUIREMENTS 

All requirements are due by August 1, 2024 (11:59 pm). Any late submission will result in a financial fee.

        *** NOTE: ALL freshmen and transfer athletes, and athletes new to UWL NCAA athletics will need to see their medical provider for a physical exam completed on the UWL physical exam form and sickle cell trait testing.*** 

        *** All new physicals for new and transfer student athletes MUST be dated within 6 months of the start of activity.  Any physicals dated prior to this will NOT be accepted. ***

        *** Physical exams expire after 5 years. Any athlete returning to utilize a 6th year of eligibility will likely need a new physical. Please contact UWLAT@uwlax.edu if you have questions regarding your physical expiration date.  

        ***ALL supporting documents (insurance cards, physicals, and sickle cell test results) must be uploaded electronically, please review ATS instruction links for file upload instructions.***

Additional requirements for ADD/ADHD and/or Heart Conditions

  • What additional requirequirements are there for individuals diagnosed with ADD/ADHD
  • Attention Deficit/Hyperactivity Disorder (ADHD): 
    •  The treatment of ADHD often involves the use of prescription drugs such as Adderall and Ritalin.  Certain classes of drugs can harm student-athletes, and can create an unfair advantage in competition; therefore, are prohibited by the NCAA.  August 2009 marked the beginning of the NCAA’s updated regulations on monitoring and documentation of ADHD in student-athletes to prevent danger/abuse of these drugs.  It is the athlete’s responsibility to submit all documentation in order to ensure eligibility. 
    • If you are currently under care for ADHD please download and review the following document as the guidelines by the NCAA. 
  • In addition to ADHD/ADD medications, there are other medications that student-athlete's may be prescribed that also may fall under the NCAA Banned Substance List.  One other commonly prescribed medication that is new to this list is albuterol inhalers.

    • If you are prescribed medications you can check if they are on the banned substance list by searching them on Drug Free Sport https://www.drugfreesport.com/
      • Click on Axis Login
      • Sign in or Create a log-in
        • If creating a log-in, fill in email and choose a password.  The website will then send you a verification code to your email. 
        • Enter verification code. 
        • It will then ask you to fill in Organization - choose NCAA Division III and a "code" - that is ncaa3. 
        • Complete User Profile.
        • This should create your account.  You can then search any medication to find out its status.

  • If you are currently prescribed any medication that falls on the NCAA Banned Substance List, please download this form and have your prescribing physician complete in it's entirety. 
    • Medical Exception Reporting Form
    • This form is also available under e-files on ATS. 
    • This form must be completed and on file in ATS PRIOR to the start of your sport's post season competition.
